Fill Out Data Transfer Form

The Data Transfer Form (DTF) is used to collect and document any information you are required to submit together with the images. Whether you need to complete a DTF depends on your project: some projects require it before a visit can be submitted, while others have no DTF requirement.

Note: The DTF in your project is customized to meet your study’s requirements and may not look like the example shown in this guide.

Access and Complete DTF

The DTF is accessible next to the images in the DICOM Viewer. Open the visit dashboard and click ‘Preview Image’ to open the image viewer.

  1. The form is visible next to the images. Fill out all required fields and click “Complete form” to complete the form, after which the form becomes read-only.
  2. Once you have completed the form, click the arrow at the very bottom to return to the visit dashboard to complete the submission.

Edit Completed DTF

Completed DTFs becomes read-only by default. Follow these steps to edit a read-only DTF.

  1. Navigate to the DTF and click the “Form is locked. Click here to unlock” link. Please note that edit permissions for completed DTFs are highly restricted. In most cases, editing will not be allowed unless specific criteria are met or elevated access is granted.
  2. Once you’ve updated the DTF, click the “Save” button to save your changes.

If you find an error in a completed DTF for an un-submitted visit, you can reset it from the visit dashboard. Be aware that doing so will clear all form fields.

  1. Click the “Reset checklist” link to reset a form.

Download PDF record

If your trial is configured to allow it, you can download a PDF copy of the completed DTF from the visit dashboard.

  1. Open the ‘Documents’ section and click download.
